The Social Worker promotes desired patient outcomes by identifying emotional, social, or environmental barriers and intervenes accordingly. Actively participates in the development of the plan of care and assists in coordinating / facilitating care elements. Conducts psychosocial assessments that encompass the patient's physical, emotional, economic and social needs. Participates in interdisciplinary and interagency collaboration and consultation.Conducts utilization review, evaluates clinical information, and communicates findings to payors within the framework of contractual and government regulations.
This position is eligible for 15% On Call Premium pay for each hour worked.
Education: Masters of Social Work with two years of post-master's social work practice required.
License / Certification: Licensed Clinical Social Worker (LCSW) by the State of Oregon preferred. May require possession of a valid Oregon Driver's License with proof of safe driving record for the past ten years and proof of insurance. Basic Life Support (BLS) certified by the American Heart Association (for Hospice position only).
Experience: Must be skilled in crisis intervention and possess high level of problem solving skills. Interpersonal communication skills and ability to function in a team environment with healthcare professionals required. Must have the ability to recognize and respond to the human dignity of those served.
